aseline;line-height:24px\">（一）符合《中華人民共和國政府采購法》第二十二條之供應(yīng)商資格條件要求，并按照《政府采購法實(shí)施條例》第十七條的規(guī)定提供以下材料：

aseline;line-height:24px\">3.具有履行合同所必須的設(shè)備和專業(yè)技術(shù)能力：提供相關(guān)證明材料或承諾書（格式自擬）；
aseline;line-height:24px\">4.參加本次政府采購活動(dòng)前三年內(nèi)，在經(jīng)營活動(dòng)中沒有重大違法記錄：提供聲明函（格式自擬）；
aseline;line-height:150%\">5.具備市政工程施工總承包叁級(jí)及以上資質(zhì)；有效的安全生產(chǎn)許可證；項(xiàng)目經(jīng)理需具備市政工程二級(jí)建造師，且未擔(dān)任其他在建建設(shè)工程的項(xiàng)目負(fù)責(zé)人或項(xiàng)目經(jīng)理。(提供資質(zhì)證書、安全生產(chǎn)許可證及項(xiàng)目負(fù)責(zé)人（項(xiàng)目經(jīng)理）注冊建造師執(zhí)業(yè)資格證和安全生產(chǎn)考核合格證書（A類或B 類））

aseline;line-height:24px\">①對(duì)列入失信被執(zhí)行人、重大稅收違法案件當(dāng)事人名單、政府采購嚴(yán)重違法失信行為記錄名單且還在執(zhí)行期的供應(yīng)商，拒絕其參與本次采購活動(dòng)。查詢渠道：信用中國網(wǎng)站（www.creditchina.gov.cn）、中國政府采購網(wǎng)www.ccgp.gov.cn）。
