如果投標人為境外注冊公司，需提供有效的公司登記注冊證明；
The bidder within the customs territory shall provide valid independent legal person business license, tax registration certificate, organization code certificate, or combined？certificate and business license. The scanned copy of the original shall be provided during bidding (the original copy for reference);
The bidder outside the customs territory shall provide valid certificate to prove valid business registration certificate.
B.投標人須提供制造商的放射性儀表《SIL2認證證書》，且證書在有效期內(nèi)。投標時需提供原件掃描件。
The bidder must provide manufacturer's “the SIL2 certificate of radioactive instrument”, and the certificate shall be within the validity period..The scanned copy of the original shall be provided during bidding.
C.本次招標不接受代理商投標。投標人應是投標貨物的制造廠商（包括制造廠商所在集團內(nèi)負責銷售的公司）。如果投標人是制造廠商所在集團內(nèi)負責銷售的公司，投標人應提供聲明，說明投標人與制造廠商隸屬同一集團，負責投標貨物的銷售工作。
Bid by Agent is not accepted.？The bidder shall be the manufacturer of bid goods, ？including？the sales company within the same group as the manufacturer of bid goods. If the bidder is the sales company within the same group as the manufacturer, the bidder should provide relevant documents to prove that the sales company is within the same group as the Manufacturer and it is responsible for the sales of the bid goods.

（2.1）投標人須提供投標人的《輻射安全許可證》，且證書在有效期內(nèi)。投標時需提供原件掃描件。

（2.2）投標人須提供制造商的放射性儀表《防爆證書》，需體現(xiàn)氣體防爆認證和粉塵防爆認證，防爆等級不低于GB/T 3836或IEC-60079 標準規(guī)定的Ex db IIC T6 Gb+Ex tb IIIC T135℃ Db，防護等級不低于IP66，且證書在有效期內(nèi)。投標時需提供原件掃描件。
The bidder must provide manufacturer's “Explosion Proof Certificate” for the radioactive instruments,and meet the Ex db IIC T6 Gb+Ex tb IIC T135℃ Db explosion-proof and IP66 protection requirements as specified in the GB/T 3836 or IEC-60079 standards, and the certificate shall be within the validity period. The scanned copy of the original shall be provided during bidding.

（3）業(yè)績要求Requirement for Bidder's reference:
2015年1月1日至投標截止日（以合同簽署時間為準），投標人所投產(chǎn)品的制造商應具有1個項目的聚烯烴裝置放射性儀表業(yè)績，且單個合同放射性儀表數(shù)量大于或等于20臺。聚烯烴裝置包括聚丙烯裝置、低密度聚乙烯裝置、高密度聚乙烯裝置、全密度聚乙烯裝置（提供的業(yè)績滿足其中一個裝置即可）。如果投標人為制造商所在集團內(nèi)負責銷售的公司，應提供該制造商的業(yè)績。
From January 1st, 2015？to the deadline for submission of the bid (subject to the signing time of the contract), the manufacturer of the products supplied？by the bidder shall have？at least one project？Radioactive Instruments？for？Polyolefin Plants？performance, and the number of Radioactive Instruments of single contract need to be greater than or equal to 20.？Polyolefin plants include polypropylene plant, low-density polyethylene plant, high-density polyethylene plant, full-density polyethylene plant. (The performance provided meets only？one of the plants).？If the bidder is a company responsible for sales within the manufacturer's group, the performance of the manufacturer shall be provided.
投標人須按規(guī)定格式提交業(yè)績表，并提交相關業(yè)績證明文件。業(yè)績證明文件包括但不限于：銷售合同復印件和用戶簽字或蓋章的到貨驗收材料。投標人所提交的業(yè)績證明文件必須至少體現(xiàn)以下內(nèi)容：合同簽署時間、合同簽署頁（國內(nèi)貿(mào)易合同應有雙方蓋章，國際貿(mào)易合同應有雙方簽字或蓋章）、項目名稱、制造商名稱、裝置名稱、貨物名稱、貨物數(shù)量。
若業(yè)績合同為年度協(xié)議，除提供年度協(xié)議外，還應提供相應的已到貨訂單，訂單內(nèi)容或編號應與年度協(xié)議相關聯(lián)。同一個年度協(xié)議下提供1個或以上的訂單及與訂單對應的到貨驗收材料均算為1個有效業(yè)績。
The Bidder shall furnish reference list in given form, and provide corresponding documentary evidence. The documentary evidence shall include but not limited to: copies of sales contract and？acceptance evidence of goods delivery signed or sealed by the user.？The？reference evidence documents furnished by？Bidder shall as least demonstrate the content as follows: Date of signing contract, contract signing page(domestic trade contracts should have signatures or seals from both parties, international trade contracts should have signatures or seals from both parties),？name of project, name of manufacturer, name of the plant, name of goods,？number？of goods？.
If the performance contract is an annual agreement, in addition to providing the annual agreement, corresponding received orders should also be provided, and the order content or number should be associated with the annual agreement. Providing one or more orders and corresponding acceptance evidence？under the same annual agreement is considered as one valid performance.
